Upon arrival, patients with dental insurance are asked to provide all relevant insurance information, including your insurance card, and to keep us updated on any changes in address, phone number, or employer.
We gladly assist in obtaining your benefits and will bill your carrier as a courtesy. However, you are responsible for your estimated portion at the time of service and, ultimately, for any remaining balance. Our staff can help answer most questions regarding your coverage.
Accepted Insurance Providers: Aetna, Cigna, Assurant, Sunlife, Ameritas, Reliance, Principal, Physicians Mutual, and United Concordia Elite Plus plan.
Patients without insurance are expected to pay for services at the time they are rendered. We accept Visa, MasterCard, and Discover. For more extensive treatment, our staff is happy to discuss additional payment options.
Checks returned by your bank are subject to a $20 processing fee. Accounts unpaid after 60 days from the date of service are subject to a $5 re-billing charge. We encourage timely payment to avoid additional fees.
Please call in advance if you need to reschedule. When you book an appointment, you reserve our time and attention. Cancellations with less than 48 hours’ notice may incur a $25 cancellation fee, which is not covered by insurance.
All payments are expected at the time of service. We also offer CareCredit, a convenient, low minimum monthly payment program for your entire family, designed to cover healthcare, elective, or cosmetic dental treatment not covered by insurance. Applications can be completed quickly in our office or online at the CareCredit website.
